Abstract: Background and Aims: Middle ear surgery requires bloodless surgical field for better operatingconditions, deep level of anaesthesia and rapid emergence. Recent studies suggest that α2agonists could provide desired surgical field, sedation and analgesia. The present study was aimed to evaluate the clinical effects of dexmedetomidine infusion as anaesthetic adjuvant during middle ear microsurgery.

Methodology: This was a descriptive cross sectional study conducted at the University of Port-Harcourt Teaching Hospital, Port- Harcourt (UPTH), South-South Nigeria, between June and September, 2014. It consisted of a set of structured, pretested questionnaires that were administered to consecutive antenatal attendees at the centre.

Abstract: The aetiology of anal fissure is not fully understood—why some heal spontaneously and others become chronic remains a mystery. In this study we compared use of sitz bath to medical therapy in the treatment of acute anal fissure on OPD basis.Seventy five patients with an acute first episode of anal fissure were randomised to receive a three weektreatment of Sitz bath with psyllium husk vs 5% lignocaine ointment vs 2% Glyceryl trinitrate ointment. After one, two and three weeks of treatment, symptomatic relief and healing rates were comparable among patients treated with sitz baths with psyllium husk and GTN ointment than those treated with lignocaine ointment. Therefore, a cheap and effective treatment of sitz bath can be used as first line instead of GTN ointment which has potential side effects.

Abstract: Malaria is a public health problem in several parts of the country. About 95% population in the country resides in Malaria endemic areas and 80% of malaria reported in the country is confined to areas consisting 20% of population residing in tribal, hilly, difficult and inaccessible areas. India contributes about 70% of malaria in South East region of W.H.O. Although annually India reports about 2 million cases and 1000 deaths attributable to malaria, there is increasing trend in the proportion of Plasmodium Falciparum as the agent. Abstract: Medical studies press unprecedented demands on the young students. Unlike the school curriculum, medical curriculum requires greater involvement of students in organized learning activities. Instead of traditional ways of teaching, new methods of teaching, learning and evaluation may inspire the students desire for learning and once the students mind is set on fire, it will find its way to provide its own fuel. Thus, the ultimate goal of self teaching and learning could be achieved. With this view in mind, we thought of conducting quiz competitions as Quiz [1] [2] is one of the various methods [3] [4] described in the literature. Studies have shown that quiz games promote active learning and provide motivational impetus. They also enhance meaningful knowledge retention by igniting interest and placing theoretical subjects in a real-world perspective. Thus, game styled quiz competitions based on curricular subjects can be an effective tool to increase interest and motivation and enhance learning is established beyond doubt.

Abstract: Emphysematous prostatitis (EMP) is a very rare inflammatory condition, characterized by localized collection of gas and purulent exudates in the prostate and carries a high mortality rate. We present a case of 62 year old man admitted with fever and chills , without any urinary complaints or co-morbidities and had none of the usual risk factors for EMP.Imaging revealed a collection within the prostatic parenchyma with air shadowsextending into right pararectal area.Patient was successfully treated with guided aspirationand broad spectrum antibiotics without any complications.

We report a case of oligodendroglioma in a middle aged male patient presenting as a rapidly growing intramedullary spinal cord tumour, which was surgically excised.

Abstract: Anatomical variations of Renal arteries are common, usually kidney is supplied by the renal artery which is the anterolateral branch of the abdominal aorta. During the regular dissection of cadavers in the Department of Anatomy, we observed no.of variations in the pattern of renal artery instead of single renal artery to the kidney. Some renal arteries showing early ramification before entering into the hilum of kidney. For some other kidneys along with main renal artery extra renal arteries going to the kidney through its hilum are accessory renal arteries and some extra renal arteries are directly going to the upper or lower pole of the kidney without entering into the hilum are aberrant arteries. As per our study 32% of the specimens showing variations. 68% are the single renal arteries irrigating the kidneys. Morphological variations of the renal arteries are common. It is better to have awareness to prevent the surgical hazards because of anatomical consideration.

Abstract: Comparison of maternal fetal outcome with active versus expectant management in PROM Material and Methods: From March 2007 to July 2008 a total of 103 pregnant women with PROM allocated randomly to expectant group A (53) and active group B (50) and fetal and maternal morbidity were compared at Government Maternity Hospital, Tirupati. Results: PROM to delivery interval in expectant group was 30.49 16.07 hours, in active group it was 17.46 6.0 hours. Caesarean delivery rate in expectant group is 5.7% and 12% in active group. In group (A) 32 % of the patients had spontaneous labour but in group (B) it is 67%.Incidence of caesarean delivery and operative delivery was 12% and 18% in active group compared to 5.7% and 5.7% in expectant group. 81.1% and 96 % of babies in both groups at birth had APGAR 7 respectively. Neonatal mortality is 1.9% in expectant group and 2% in active group. Maternal morbidity in expectant cases was 3.81% and in active cases 4%. Neonatal morbidity in expectant group was 20.6% and in active cases was 18%.

Abstract: The defect which is associated with loss of eye can lead to major physical, mental and emotional disturbance. Treatment of facial deformity is a difficult task in which procedure for each patient is very skilled and précised. Visual judgment alone will not be perfect. Here we report a case of 45-year-old female patient with meningiosarcoma of the right eye, undergone surgical enucleation and was rehabilitated by spectacle with magnet retained silicon orbital prosthesis in order to accomplish perfect fit and aesthetics. Abstract: Breast cancer is a disease condition that is associated with alterations in the body's physiological homeostasis. Bone metastases are common in advanced cancer and cause considerable morbidity and mortality. The study aimed to assess the serum levels of alkaline phosphatase activity and the bone minerals in breast cancer. A total of 80 subjects within the ages of 20-65 years were investigated (40 breast cancer subjects and 40 age-matched apparently healthy female subjects as controls). Serum calcium was estimated using calcium kit from Randox Laboratories, inorganic phosphate and magnesium were estimated using kits from Biosystems while the estimation of serum alkaline phosphatase activity was done using King Armstrong's method.

Abstract: Toxic epidermal necrolysis is a common life threatening dermatological emergency. It occurs most often as an adverse drug reaction due to drugs like NSAIDS, antibiotics, anticonvulsants, quinolones. It is acute in onset and involves integumentary system and other systems having high mortality. This condition necessitates intense management because of acute skin failure characterised by confluent purpuric and erythematous macules evolving in to flaccid blisters and epidermal detachment predominantly on the trunk and upper limbs and associated with mucous membrane involvement. Awareness regarding drugs causing TEN is known but Quinine as an etiological factor is never reported as cause for TEN in existing literature. Here we are reporting three cases which developed signs and symptoms of TEN shortly after initiating Quinine therapy. The cause in this cases was found to be Quinine which was given to patients undergoing treatment for Malaria. Usage of Quinine in cases with Falciparum Malaria is still being practised by many physicians and awareness of this dreaded complication is important.

Abstract: Chylothorax is a rare clinical entity characterised by a milky white fluid with increased triglyceride levels and chylomicrons in the pleural cavity. The commonest aetiology is malignancy and trauma. Persistent leak leads to significant loss of essential proteins, immunoglobulins, fat, vitamins, electrolytes and water and needs a prompt treatment. Bilateral idiopathic Chylothorax as observed in the present case is a rare entity.

Abstract: Periapical lesions of endodontic origin may develop asymptomatically and become extensive. Treatment options to manage such lesions range from non-surgical root canal treatment and/or apical surgery to extraction. A high percentage of complete and partial healing of extensive perapical lesions following conventional endodontic therapy has been reported. The case reports in this article describe the management of such lesions by conventional endodontic therapy. Triple antibiotic paste and calcium hydroxide were used as intra canal medicaments. The two and three year follow up radiographs of the cases showed progressive healing of periapical lesions. Abstract: Supernumerary roots are uncommon in primary teeth. Primary mandibular second molars normally have two roots, one mesial and one distal. Rarely an additional third root is seen and when this extra root is situated distolingual to the main distal root it is referred as Radix Entomolaris and when it is present mesiobuccal to mesial root it is called Radix Paramolaris. The knowledge of the presence of additional roots and root canals is very essential for the successful endodontic treatment. This report presents an unusual case of bilateral Radix Entomolaris in primary mandibular second molars in a 5-year-old male patient. Abstract: Medullary thyroid carcinoma (MTC) originates from the thyroid parafollicular cells and accounts for 3% to 10% of all thyroid malignancies. Approximately 84% of cases are sporadic. A case of 45 yrs male patient presented with thyroid nodule and difficulty in swallowing.Fine-needle aspiration biopsy (FNAB)had been performed preoperatively subsequently it was reported as follicular neoplasm .In histopathology it is reported as medullary carcinoma of thyroid. IHC on sections show Thyroglobulin is negative , Calcitonin is positive and it is confirmed as medullary carcinoma of thyroid. Since it is a rare case ,we present this case.

Abstract: Objectives: To determine and compare Total Antioxidant Capacity (TAC)of saliva in children undergoing active orthodontic treatment with removable and fixed orthodontic appliances before and after treatment. Methods: Total antioxidant capacity of saliva was investigated in 46 healthy children aged 8-14 years. The children divided into three groups. Group A treated with fixed orthodontic appliance , group B treated with removable orthodontic appliance and group C receive no treatment. Saliva samples(2mm) were collected. in different time intervals: before the orthodontic treatment, three months and six months after treatment. Salivary total antioxidant capacity was measured by spectrophotometric method.
